Continuous Glucose Monitoring (CGM)
CGM Devices
A continuous glucose monitoring system (CGM) measures blood glucose levels in real time throughout the day and night. By wearing a continuous glucose monitor system you will have a better understanding of what your blood glucose levels are doing from moment to moment. After the initial calibration, the CGM measures your blood glucose every 1-10 minutes. This makes the CGM a robust and life changing piece of technology for those with diabetes. CGM devices come in three parts - the sensor, the transmitter, and the receiver (which for most folks ends up being a smartphone).
The sensor is inserted just below the skin and worn for up to two weeks, thereby eliminating the need for daily finger pricks. The transmitter continuously sends blood sugar readings to the receiver as often as every one minute with the FreeStyle Libre or every five minutes with the Dexcom G6 . Either way, you’re able to get a real-time blood sugar reading at virtually any time in the day you require one. Plus, most CGM devices will warn you with an alarm should your blood sugar drop dangerously low or spike too high. But the real advantage for you and your physician is the ability to identify blood sugar trends as they happen, including gauging the impact food and physical activity have on your blood sugar in real time. This can help generate more accurate decisions in terms of insulin dosing and other medications. If fact, CGM’s have been proven to lower A1C levels in most individuals who use them.
So, with all these great advantages why wouldn’t everyone be better off with a CGM solution? There are a number of reasons.
Glucose Meter vs CGM
Everyone’s diabetes management program is unique. We all live different lives with different economic situations, physical activity routines, treatment programs, and blood sugar testing requirements. For many people a glucose meter is actually a better choice than a CGM device. Let’s look at some of the factors to consider when making your decision.
Testing Schedule & Blood Sugar Control
If you’re effectively managing blood sugar using a glucose meter and only require a few finger pricks each day, there’s probably no real advantage to switching to a continuous glucose monitoring device. Why fix it if it’s not broken? However, if you are having a hard time maintaining control over your blood sugar and are experiencing frequent lows or highs, a CGM device might help you gain greater control thanks to the real-time, anytime readings. You’ll likely be able to identify trends and pinpoint the variables that are throwing things out of whack.
Cost & Insurance
Sometimes it comes down to brass tax. CGM devices can be a lot more expensive than glucose meters. Additionally, the test strips and lancets used with a glucose meter cost a lot less than the sensors and transmitters you’ll need to replace regularly for a CGM device. On the plus side, many insurance providers will cover at least a portion of your CGM needs based on your physician’s prescription. However, if it is determined that you’ve been successfully managing your diabetes without the use of a CGM device, your insurance company might consider it a non-necessity and deny or limit coverage despite your physician’s prescription. For many people, cost weighs heavily in the decision. If the cost outweighs the benefits of switching, or just puts too great a burden on your financial situation, you’re likely better off sticking with a glucose meter.
You Wear A CGM Sensor All Day And All Night
For the most part CGM sensors and transmitters are pretty unobtrusive, worn on the arm or abdomen. Nevertheless, they are on your body day in and day out. For some people, this can be a constant reminder that they have diabetes. For others, it can be uncomfortable when playing sports or doing certain physical activities. If you’re not ready to wear a sensor throughout the day, a CGM device may not be for you.
Inconspicuous Testing When In Public
One big advantage CGM devices have over glucose meters is that they make those daily blood sugar tests far less conspicuous. Instead of being required to carry test strips and prick your finger out in public, a simple glance at your CGM receiver or wave of your smartphone over the transmitter gives you an instant reading anytime. Chances are no one will even notice you’re doing it when out in public.
